Does anyone know if/where/when Grandis and Paré's Multiple categories of generalised quintets has been published?

I believe the paper itself has never been published (I do not know why). However, much (if not all) of the content appears in Grandis's book Higher Dimensional Categories: From Double to Multiple Categories.
