I had a look for Lack's thesis recently, with no luck. I think easiest would be to email him about that one.

Alex Hoffnung was my student but I don't recall seeing his "Notes on enrichment".
For what it's worth, I just tweeted Alex to see if he has a copy of the notes.
I have a copy of Carmody's thesis. Not sure why it's not available anywhere online.
Is Carmody contactable? Any conceivable objections to keeping a copy on a computer accidentally visible from the rest of the internet (à la Andrew Ranicki's "personal collection" of papers)
I haven't tried. I got my copy from someone else who happened to have it.
@Théo Alex sent me a copy of the notes on enrichment! If you drop me an email at one of the addresses listed at my nLab page, I can send it on to you. I will also upload the file to the nLab, which Alex has agreed to, so future generations can benefit.
