KOCK, Limit monads in categories, Aarhus Univ. Math. Inst., pre- print séries n° 6, 1967 but I can't find digital evidence of this preprint's existence, even though Anders has a slightly earlier 1966 preprint still on his site) other than going to Houston, Chicago, or Aarhus?
